Health Trainer

One post is available at 30 hours per week

(Salary: NJC Scale 3, £15,725 - £16,830 pro rata)

 

 

An exciting opportunity has arisen for Sharrow Community Forum to appoint a Health Trainer to join our local community health team.  The Health Trainer will work with local GP Surgeries across Sharrow to provide them with 1:1 support to help individuals and their families eat healthily and lead an active lifestyle.

 

Do you have experience of working with local communities and the ability to motivate people to make positive changes with their behaviour?  Would you like the opportunity to help address health inequalities and improve the long-term health of communities?  If yes, then this could be the perfect job for you!

 

The post holder will:

 

·        Promote positive health and well being messages with local people

 

·        Identify and support individuals to develop a personal health plan

 

·        Encourage and facilitate long-term positive lifestyle changes

 

·        Work within GP surgeries to attract new clients

 

·        Develop a social prescribing project with local GP surgeries

 

 

Knowledge of Sharrow is essential, so applications from residents within Sharrow are particularly welcomed.  The ability to speak a community language is desirable.

 

We are looking for someone who enjoys a challenge and has the ability to communicate effectively with a wide range of people both verbally and in writing.  Full training is available for successful candidates and the Health Trainer will form part of the wider Sheffield Health Trainer network.

 

The work is being delivered in partnership with the Sheffield Primary Care Trust as part of the Sharrow Enhanced Public Health Programme.
